Reporting non-osteopathic CME

Here's how to obtain credit for CME not automatically reported to the AOA.

Category 1-A and 1-B credit is osteopathic and does not need to be self-reported to the AOA. Category 1 CME event organizers will report your CME credits to the AOA within 90 days of the program.

Acceptable Category 2 credit includes AMA-accredited or AAFP-approved credits. These credits are typically listed on the certificate of attendance as being:

AMA PRA Category 1 Credit™
Accredited by: Accreditation Council for Continuing Medical Education (ACCME) or
Approved by: American Academy of Family Physicians (AAFP)

As we approach the end of the 2019-2021 CME cycle, we are experiencing a high volume of CME submissions; please anticipate an extended processing time. A notification will be sent via email when any changes are made to your CME Report. To expedite tracking of your CME activity, we encourage you to utilize the new CME Self-Reporting Tool.

Reporting process

Use the forms below to report CME activities not automatically reported by AOA-accredited CME sponsors and for special CME requests. You can report CME by uploading attachments to the AOA CME Self-Reporting Tool (Login Required), or by emailing attachments to cme@osteopathic.org. Please include your AOA number on all documentation. A notification will be sent by email when your CME Report has been updated. Include your name and AOA number with all submissions.

Submit copies of the certificates of attendance. Do not send the originals.
Include your contact email or phone number in case there are questions.
Please do not submit the same certificates of attendance multiple times, as this slows the posting process.

Credits not applicable for 2019-21 CME cycle

Credits earned or acquired prior to Jan. 1, 2019
Credits not accredited by AOA or ACCME, or approved by AAFP

Non-osteopathic CME

2019-2021 Individual Certification Form
This form is for individual DOs to report non-osteopathic CME activities and other activities which have not already been reported by sponsoring organizations.
Formal Request for Category 1-B Credit
Use this form to request AOA Category 1-B credit for allopathic-sponsored CME programs.

Home study CME

Home Study Form 2019-2021
This form is for individual DOs to document reading of recognized scientific journals, listening to approved audio tapes, and other approved home study courses and programs under the criteria described for Category 2-B CME.

Healthcare facilities CME

2019-2021 Healthcare Facility Education/Committee and Department Meetings
This form is for reporting attendance at AOA-accredited and non-AOA accredited healthcare facility committee and departmental meetings concerned with the review and evaluation of patient care. For Category 1-B credit.

CME sponsors

Roster of Attendance
Submit this form to report and designate CME credits for DOs who have completed CME programs sponsored by your organization.
Speaker Roster
Use this form to report credits for DOs who lecture or serve as speakers at AOA-accredited CME programs.
REMS Pre-Approval Form
This form provides instructions for Category 1 CME sponsors requesting approval for an ER/LA REMS activity.
REMS Post-Activity Form
This form must be submitted by Category 1 CME sponsors to the AOA to report the results of an ER/LA REMS activity.
